Overview

Chestnut Family Health Center, a division of Chestnut Health Systems is is seeking a Licensed Practical Nurse to work  full-time  as a Nurse Care Coordinator in our federally qualified family health center in Granite City, IL.

Responsibilities

  • Provide care coordination services for patients.
  • Communicate test results to patients/and or families.
  • Follow-up with patients, review plan of care and progress toward goals.
  • Manage referrals.

Qualifications

  • Must be a Licensed Practical Nurse, licensed in the state of Illinois.
  • High School diploma, nursing school certificate, and five (5) years post license experience in family practice, pediatrics, health center, behavioral health or other applicable medical setting also required.
  • Prior experience in care coordination or case management preferred.
  • Must be skilled in providing care coordination for patients who have co-occurring primary care and behavioral health conditions.
  • Must have knowledge of community medical/behavioral health resources and possess the interpersonal skills to help patients access resources, foster professional working relationships with staff from these resources and coordinate care across internal/external providers and organizations.
  • Must be team-oriented and willing to work in a multidisciplinary team environment.
  • Excellent communication (written and verbal), time-management, and organizational skills are required.
  • Requires basic computer skills and experience to include MS Word, Outlook, e-prescribing and an electronic medical record (EMR).
  • Satisfactory completion of Chestnut’s background checking process.
