    Chestnut Health Systems is seeking full-time Case Managers for our Bloomington, IL facility.

    Responsibilities

    • Act as a wellness coach for patients involved in behavioral health programming, including using motivational interviewing to engage patients in wellness activities.
    • Complete evaluations as directed.
    • Provide education on wellness and treatment services to potential referral sources and project participants.
    • Provide pre-engagement sessions to selected clients.
    • Complete required paperwork on each client, including but not limited to Chestnut’s opening paperwork, evaluations, and progress notes. Required documentation is to be submitted on the day services are provided unless otherwise approved by supervisor.  Provide ongoing monitoring of services contained in the individualized treatment plan to ensure timely provision of services.
    • Provide referral, linkage and discharge to recommended Chestnut wellness and treatment services and community resources, including requests for client assistance funds.
    • Provide ongoing support and advocacy for clients especially around wellness.
    • Document services to clients in accordance with established agency policies and procedures and in compliance with applicable grant requirements.
    • Function as a team member, attend meetings and client staffings, participate in program development activities and provide support and backup to other team members as necessary.
    • Understand your role as an emissary of Chestnut Health Systems to referral sources, collaborators and community members. Present yourself as collaborative, helpful and resourceful.
    • Attend weekly supervision and utilize supervision appropriately.
    • Attend all mandatory Chestnut trainings and CEU requirements if applicable.
    • Participate in department training, educational activities and intra-agency meetings in order to achieve/maintain professional development and upgrade knowledge/skills.
    • Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery; including utilizing recovery language in all applicable documentation.
    • Assume leadership role as assigned.
    • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.

    Qualifications

    • Must have a bachelor’s degree and five (5) years’ substance abuse and/or mental health experience.
    • Must have a valid driver’s license, private automobile insurance and be insurable.
    • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
    • Satisfactory completion of Chestnut’s background checking process.
