    Chestnut Health Systems is seeking a PRN Case Manager to work within our Crisis Stabilization Unit at our Bloomington, IL facility.

    Responsibilities

    • Provide complete assessments, including level of functioning of clients’ needs.
    • Meet with clients and their support networks to develop an individualized treatment plan.
    • Complete required paperwork on each client, including but not limited to Chestnut’s opening paperwork, mental health assessments, individual treatment plans, and progress notes.
    • Provide ongoing monitoring of services contained in the individualized treatment plan to ensure timely provision of services.
    • Provide for referral, linkage and discharge to recommended CHS services and/or community resources, including requests for client assistance funds.
    • Provide advocacy, both client and system centered, to ensure that basic rights of the chronically mentally ill are protected.
    • Provide appropriate mental health services/interventions, including crisis intervention, case management, individual and group counseling, family counseling and/or consultation and/or client centered consultation.
    • Assist clients with daily activities of the crisis residential unit.
    • Provide basic first aid to clients as indicated.
    • Document treatment to clients in accordance with established agency policies and procedures and in compliance with applicable grant requirements; required paperwork to be completed and submitted in a timely manner. Required documentation is to be submitted on the day services are provided unless otherwise approved by supervisor.
    • Attend 100% of monthly, required Rule 132 supervision if not an LPHA and ensure supporting documentation of same.
    • Attend all mandatory Chestnut trainings and CEU requirements if applicable.
    • Participate in program specific training, educational activities and intra-agency meetings in order to achieve/maintain professional development and upgrade knowledge/skills.
    • Develop a working knowledge of the Mental Health Code and Confidentiality Act and its application to child services.
    • Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery; including utilizing recovery language in all applicable documentation.
    • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.

    Qualifications

    • Must have a high school diploma or equivalent and five (5) years’ supervised mental health experience.
    • Or a bachelor’s degree in counseling, social work, education, psychology or related human services field.
    • Must have a valid driver’s license, private automobile insurance and be insurable.
    • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
    • Satisfactory completion of Chestnut’s background checking process.
