    Wellness Connections is looking for a licensed clinician (LCSW, LCPC) to serve as a Targeted Care Specialist.

    Responsibilities

    • Conducts street outreach to develop relationships and build rapport with clients.
    • Use supportive, strengths-based engagement strategies, including motivational interviewing and harm reduction principles.
    • Assist individuals with accessing appropriate mental and physical health services, housing, substance use treatment, employment, complete clinical follow ups after discharge from hospitalizations and any other services that assist an individual to improve his/her quality of life.
    • Primary point of contact with program participants and provides screening, triage, crisis intervention, and referral to supportive services, with the goal of assisting individuals with moving towards their recovery goals.
    • Serve as a clinical resource for non-licensed staff.

    Qualifications

    • Must have a Master’s degree in a human services field and qualify as a Licensed Practitioner of the Healing Arts (LPHA) in the state of Illinois.
    • IL licensure of LMFT, LCSW or LCPC required.
    • Must have a valid driver’s license, private automobile insurance and be insurable.
    • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
    • Satisfactory completion of Chestnut’s background checking process.
