Chestnut Health Systems is seeking a full-time Integrated Care Specialist for our Integrated Wellness program at our Granite City, IL facility, servicing the Metro East and St. Louis, MO areas. Integrated Wellness provides group services focused on skill building, wellness education, and therapy.

The Integrated Care Specialist is a community based position that focuses on building rapport with clients to help them gain different skills to live independently. An ideal candidate will use supportive, strengths-based engagement strategies, including motivational interviewing and harm reduction principles.

• Assist individuals with accessing appropriate mental and physical health services, housing, substance use treatment, employment and any other services that assist an individual to improve his/her quality of life.
• The Integrated Care Specialist is the primary point of contact with program participants and provides screening, triage, crisis intervention, and referral to supportive services, with the goal of assisting individuals with moving towards their recovery goals.
• Candidate must be organized and complete all necessary paperwork to track client progress, and meet with established clients on a weekly basis to review, evaluate, and support recovery goals.

• Must have a high school diploma or equivalent and five (5) years’ supervised mental health experience.
• Or a bachelor’s degree in counseling, social work, education, psychology or related human service field.
• Or a bachelor’s degree in any other field with two (2) years’ supervised clinical experience in a mental health setting.
• Or a certificate of psychiatric rehabilitation from a DHS-approved program plus a high school diploma or equivalent and two (2) years’ experience in providing mental health services.
• Or a Certified Recovery Support Specialist (CRSS) in good standing with the State of Illinois.
• Must have a valid driver’s license, private automobile insurance and be insurable.
• Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
• Satisfactory completion of Chestnut’s background checking process.
