Chestnut Health Systems is seeking a full-time Case Manager to promote integrated wellness to work primarily daytime hours Monday through Friday for our Bloomington, IL location. This might be the job for you if you enjoy working with underserved populations as a part of an innovative program utilizing evidence based practices to positively influence the physical and behavioral health of our patients. Providing self-management support, enhancing self-efficacy, assisting with healthy behavior change, and promoting appropriate use of healthcare services are some of the crucial and personally rewarding contributions you would make to patients and integrated primary and behavioral healthcare teams.

Integrated Wellness Coach Responsibilities: 

  • Assist with program awareness campaign efforts including promoting and enrolling patients into program.
  • Conduct program related patient assessments.
  • Build and foster trusted, patient centered relationships to keep patients engaged in primary care services.
  • Reinforce health education messaging to patients as directed by primary and behavioral healthcare providers, and registered nurses.
  • Collaborate with patients and care team to set, evaluate, and adjust patient-centered wellness goals.
  • Facilitate wellness groups on healthy eating, exercise, tobacco cessation, etc.
  • Track continuously patient progress; reporting updates to care team.


Integrated Wellness Coach Job Qualifications: 

  • Minimum of a high school diploma or equivalent with at least five (5) year’s supervised mental health experience; or a Bachelor’s degree in counseling, psychology, social work, or related human service field.
  • Requires basic computer skills and MS Work, MS Excel, Outlook, Adobe Acrobat, and electronic medical record experience.
  • Must have a valid driver’s license, private automobile insurance and be insurable.
  • Strong desire to help patients with chronic medical conditions, mental health, and substance use disorders.
  • Obtain a NCAA accredited personal training certification within one year of hire date if not currently certified.
  • Satisfactory completion of Chestnut’s background checking process.

 Preferred Qualifications: 

  • Experience in developing, planning, implementing, facilitating, and evaluation in-person and virtual healthy eating and fitness activities and events.
  • Master’s degree in counseling, psychology, social work, health science, health promotion or related humans service field.
