Overview

Chestnut Health Systems is seeking a full-time Case Manager (intake department) to work at our Maryville, IL facility, servicing the Metro East and St. Louis, MO areas.

 

A shift differential is available for weekend, evening, and overnight hours.

Responsibilities

  • Support and model Recovery Value of Hope by maintaining a strength-based approach, encouraging clients and advocating for their success. Help to normalize the treatment process during both assessment and admission process.
  • Support and model Recovery Value of Respect by speaking to clients, referral sources and colleagues assertively, answering their questions to the best of our ability, treating others with unconditional positive regard and dignity and including families or other support in the assessment and recommendation process by asking questions and coordinating information.
  • Support and model Recovery Value of Empowerment by offering support and guidance yet allowing clients to make and learn from their own choices without being punitive, avoiding power struggles, including clients/families input into recommendations.
  • Support and model Recovery Value of Health/Wellness by modeling and encouraging clients to engage in healthy eating, physical exercise and healthy coping strategies. Take responsibility for the cleanliness of the building.  
  • Support and model Recovery Value of Spirituality/Connectedness by maintaining positive and healthy relationships with referral sources, colleagues, clients and their families, encouraging teamwork and supporting colleagues and clients. Should help with warm hand-offs to program clinical staff. 
  • Demonstrate understanding of adolescent development and trauma throughout interactions with clients and recommendations made.
  • Conduct intake and pre-admission screenings in a responsive manner according to referral source/client need. Complete documentation in a timely manner. 
  • Conduct diagnostic and assessment evaluations with clients and their families and write bio-psychosocial reports to address diagnosis and patient placement criteria as needed. Complete Columbia Suicide Severity Risk assessments according to policy and appropriateness.  Assessments may be done at off-site approved locations. 
  • Maintain regular contact/support with clients/families and referral sources during the pre-admission/waiting list process, including encouraging support in home community, preparing necessary paperwork for admission and providing updates regarding potential admission dates.
  • Complete admission appointments into treatment services, including preparing paperwork, permissions, coordinating with reception, financial intake, nursing and clinical staff, keeping the family informed and engaged in a welcoming manner.
  • Monitor and document scheduled and random urine screens for same gender clients.
  • Provide transportation to clients on an as-needed basis in order to access services or to return home.
  • Facilitate educational skills groups/presentations on various topics as requested/needed by the outpatient program and/or general public.
  • Develop and foster professional working relationships with referral sources by conducting face to face meetings as needed and checking in on their satisfaction. This might also include providing referral sources with updated information on Chestnut programming. 
  • Maintain accurate and timely records of client and all services provided (including treatment plan development and continuing stay reviews, assessment and discharge reports and progress notes) within assigned program according to policy and regulations.
  • Attend and participate constructively in all program meetings, staff meetings and supervision as assigned.
  • Participate in program and staff development activities, including initial training and competencies, mandatory trainings and achieving GAIN certification.
  • Provide court testimony when requested.
  • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
  • Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.
  • Perform other duties as assigned.

Qualifications

  • Must have a Bachelor’s Degree in a helping profession.
  • Must obtain Certified Alcohol and Other Drug Abuse Counselor certification (CADC) within two (2) years of hire date.
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook and Adobe Acrobat.
  • Prefer experience with an electronic medical record (EMR).
  • Individual must have a valid driver's license, private automobile insurance and be insurable.
  • Satisfactory completion of Chestnut’s background checking process.
