Chestnut Family Health Center, a division of Chestnut Health Systems, seeks a full-time Behavioral Health Clinician for our Granite City, IL facility, servicing the Metro East and St. Louis, MO areas. May be required to work some evenings and weekends as needed.

Our team is seeking a licensed therapist to provide behavioral health services in an integrated primary and behavioral health care setting. An ideal candidate would have experience with trauma-informed care, a desire to work with a diverse client population, and be confident in engaging in the provision of short term, solution focused therapy, motivational interviewing, CBT, and SBIRT interventions.


 Duties include:

  • Building relationships with Chestnut Family Health Center patients.
  • Providing comprehensive screening, assessment, and brief intervention for behavioral health needs.
  • Treatment plan development, specifically focusing on reducing symptoms of depression.
  • Collaborating with Providers, Health Navigators, and other Health Care staff to ensure comprehensive care.
  • Providing group, individual, and family therapy.
  • Providing support to patients during primary or psychiatric prescriber appointments.
  • Advocating for patient needs.
  • Providing psychoeducation to patients and natural supports.
  • Crisis resolution.
  • Providing coordination and referral to other service providers within the agency.
  • Participating in team education focused on behavioral health needs and interventions.
  • Coordinating with health navigators to address social determinants of health (SDOH) needs.


  • Must have a master’s degree in a human services field and qualify as a Licensed Practitioner of the Healing Arts (LPHA) in the state of Illinois. 
  • Illinois licensure of LMFT, LCSW or LCPC required. 
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
  • Must have a valid driver’s license, private automobile insurance and be insurable (when applicable).
  • Satisfactory completion of Chestnut’s background checking process.
