Overview

Chestnut Health Systems is seeking a full-time Clinician for our Supported Housing Program to work primarily daytime hours Monday through Friday with occasional evening hours at our Bloomington, IL location.

Responsibilities

  • Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, IM-CANS including applicable modules and other applicable assessments, Health Risk Assessment/Physical Health Assessment (HRA/PHA), progress notes and any other assigned requirement:
    • 100% of IM-CANS to be completed prior to due dates, and
    • Progress notes and any other required data entry or paperwork submitted within forty-eight hours of the delivery of the service.
  • Services primarily to include counseling, community support, case management, and medication training and monitoring; and services to support systems, including maintaining regular groups and individual service to consumers.
  • Services that help maintain a clean environment, including encouraging, modeling, and at times completing cleanliness tasks, to ensure a safe, clean environment.
  • Complete, maintain in working order and submit accurate and timely documentation, as assigned, to meet housing funder requirements, including but not limited to, tenant recertification attendance/census documentation.
  • Conduct and complete initial tenant applicant interviews; oversee move-ins/move-outs and complete associated documentation; work in conjunction with facility, purchasing and clinical staff; ensure all required city and county registrations are submitted.

Qualifications

  • Must have a high school diploma or equivalent and five (5) years’ social service or peer experience; or high school diploma or equivalent and be a Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS) in good standing with the State of Illinois; or have a Bachelor’s degree.
  • Bachelor’s degree and Certified Alcohol and Drug Counselor (CADC) certification; or a Master’s degree preferred.
  • Requires basic computer skills and experience to include MS Word and the use of an electronic medical record (EMR).
  • Must have a valid driver’s license with a good driving record, private automobile insurance and must meet qualifications for agency auto insurance coverage.
  • Satisfaction completion of Chestnut’s background checking process.
