Overview

Chestnut Family Health Center, a division of Chestnut Health Systems, is seeking a full-time Case Manager for our Bloomington, IL facility.

 

Chestnut Health Systems is a leading provider of behavioral and primary health care services. Over four decades, Chestnut has grown to over 700 committed, compassionate employees providing substance use, mental health and primary care treatment to patients in Central and Southern Illinois. We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers. Chestnut also conducts research and training throughout the U.S.A.

Responsibilities

  • Participate in all required staff development activities, including necessary trainings, supervision, and staff meetings.
  • Provide care coordination services for patients, including the coordination of assessment and treatment information with patient’s primary care, behavioral care and psychiatry providers as well as with families, referrals sources, schools, and/or other professionals, with periodic guidance and oversight.
  • Complete timely documentation of all activities in the patient record.
  • Support interventions consistent with the following treatment philosophies: patient centered, trauma informed, recovery oriented, and strength based.
  • Develop and foster professional working relationships with referral sources.
  • Remove transportation as a barrier to accessing care by providing transportation for patients to facilitate clinical appointments, or by assisting in connecting patients to transportation resources.
  • Participate in program development activities to improve services to patients and community, including making regular recommendations regarding the improvement of service. May include helping develop training protocols for new staff and/or participating in professional audits.
  • Operate professionally within a multi-disciplinary team setting and provide guidance to less experienced staff/interns; includes consistent modeling of the Mission, Vision, and Values of Chestnut Health Systems.
  • Function as a team member by providing support and back up to staff as necessary.
  • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.

Qualifications

  • Must have a high school diploma or equivalent and five (5) years’ supervised behavioral health experience; or a bachelor’s degree in counseling, social work, education, psychology or related human service field; or a bachelor’s degree in any other field with two (2) years’ supervised clinical experience in a behavioral health setting. 
  • Bachelor’s degree and five (5) years’ supervised mental health experience; or a master’s degree in counseling, psychology, social work or related field with less than 1,000 hours of supervised mental health experience preferred.
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR). 
  • Must have a valid driver’s license, private automobile insurance and be insurable. 
  • Satisfactory completion of Chestnut's background checking process.
