Lighthouse Institute, a division of Chestnut Health Systems, is seeking a full-time Linkage Manager for our Chicago, IL facility.


Chestnut Health Systems is a leading provider of behavioral and primary health care services. Over four decades, Chestnut has grown to over 700 committed, compassionate employees providing substance use, mental health and primary care treatment to patients in Central and Southern Illinois. We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers.   • Attain GAIN/ABS certification within specified time period, maintain specified performance levels and conduct interviews, proctor self-administered measures, and collect bodily fluids as needed to gather follow-up data.
  • Implement motivational interviewing and other interventions in special projects as assigned.
  • Participate in quality assurance and monitoring procedures for ensuring intervention and data quality and integrity; must meet and maintain quality assurance standards.
  • Learn applicable software and enter study data into the appropriate software.
  • Locate clients, collaterals, and other individuals in an effort to maintain on-going contact as outlined in projects.
  • Participate in case review meetings.
  • Provide study information to potential referral sources and research participants.
  • Travel off-site to conduct home visits, meet with clients, collaborate with outside agency contacts as outlined in projects and network to expand available agency contacts. 
  • Willingness and ability to be on-call 24-hrs/7days a week for project related tasks.  This may include linkage related tasks in the office or off-site locations at night and on weekends.
  • Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
  • Maintain and protect confidentiality of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.


  • Minimum of a high school diploma and demonstrated orientation in working with study population required. 
  • Associate’s degree or experience working on longitudinal research projects with study population preferred. 
  • Excellent time management, communication skills, and computer literacy required.
  • Satisfactory completion of Chestnut’s background checking process.
