Overview

Chestnut Family Health Center, a division of Chestnut Health Systems, is seeking a full-time Integrated Behavioral Health Clinician for our Bloomington, IL primary care facility.

Responsibilities

  • Provide services identified in individuals’ treatment plan that are consumer driven and integrates evidence-based practice concepts, including motivational interviewing and therapeutic alliance.
  • Actively participate in Quality Improvement/Quality Assurance related activities and program development activities.
  • Promote the recovery model of treatment including hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery.
  • Provide for advocacy, and referral and linkage to recommended community resources.
  • Assist patients to identify individuals’ strengths, resources, preferences, choices and natural supports.
  • Actively participate as a team member in multidisciplinary team meetings and patient case staffings.
  • Provide individual, group, family and collateral interventions.
  • Provide appropriate crisis intervention.

Qualifications

  • Must have a master’s degree in a human services field and qualify as a Licensed Practitioner of the Healing Arts (LPHA) in the state of Illinois. 
  • Illinois licensure of LMFT, LCSW or LCPC required. 
  • Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).
  • Must have a valid driver’s license, private automobile insurance and be insurable (when applicable).
  • Satisfactory completion of Chestnut’s background checking process.
