Overview

Chestnut Health Systems is seeking three full-time Case Managers to work at our Granite City and Belleville, IL locations servicing the Metro East and St. Louis, MO areas.

Over four decades, Chestnut has grown to over 700 committed, compassionate employees providing substance use, mental health and primary care treatment to patients in Central and Southern Illinois. We provide fully integrated care for all of our patients by combining behavioral health care services with our community-based primary care health centers. Chestnut also conducts research and training throughout the U.S.A.

Responsibilities

  • Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, IM+CANS, continued stay reviews, progress notes, and any other funder requirements:
    • Screen and assess for appropriate integrated care services including substance use, mental health, and primary care.
    • 100% of assessments and continued stay reviews to be completed prior to due dates.
    • Progress notes and any other required data entry or paperwork submitted within forty-eight hours of the delivery of the service.
    • Complete and submit grant required documentation reports thoroughly and timely.
  • Provide services identified in treatment/recovery/safety plan:
    • Especially specialized case management services including transportation, home-based, and virtual clinical services as needed to address barriers to consistent treatment attendance and engagement.
    • Individual, Family, and Group Counseling as recommended.
    • Utilize evidence-based practices including Community Reinforcement Approach (CRA), and motivational interviewing.
    • Encourage family involvement with frequent correspondence and psychoeducation services.
    • Provide ongoing support for clients throughout treatment and discharge planning.
    • Provide services in the environment that best meets the needs of the family.
    • Facilitate random client drug screens as recommended for treatment and required by the funder.
    • Provide a supervisor assigned level of services, consistently.
  • Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness by assisting clients/families with regaining control over their own lives through self-directed recovery, including utilizing recovery language in all applicable documentation.
  • Provide for referral, linkage, and advocacy to recommended Chestnut and other community resources:
    • Consult with co-workers and DCFS personnel to coordinate effective referrals.
  • Promote positive interactions within Chestnut and with the community, especially with Intact Family Personnel, DCFS, Advocates for Human Potential, Inc., and Madison and St. Clair County Probation, to sustain a favorable program reputation.
  • Attend meetings and staffings, participate in program development activities and provide support and backup to other team members as necessary, including:
    • Attend and ensure supporting documentation of 100% of monthly, required supervision.
    • Attend all mandatory Chestnut trainings, utilizing Relias when applicable.
    • Attend all meetings and trainings required by the funder such as multi-disciplinary staffings, court hearings, probation meetings, and evaluation meetings.
  • Provide services in an immediate access model by assessing for current needs and providing for crisis intervention when indicated.
  • Function as a team member and provide support and backup to other team members as needed.
  • Promote Chestnut’s culture of customer service through the adherence to Chestnut’s behavioral standards for customer service.
  • Maintain and protect confidentiality of all organizational information gained while performing job responsibilities including, but not limited to, financial and consumer information.
  • Provide other duties as assigned, required or specifically negotiated with supervisor.

Qualifications

  • Must have a minimum of a high school diploma or equivalent with at least 5 years relevant experience in a helping profession or college and experience combined;
  • Or have a bachelor’s in a social service area or a Certified Alcohol and Drug Abuse Counselor certification (CADC), and obtain a (CADC) within 2 years of hire.
  • Experience working with families in community settings preferred.
  • Must be willing to be trained in Community Reinforcement Approach (CRA) and achieve CRA certification within 3 months following training.
  • Must have a valid driver’s license, private automobile insurance and be insurable.
  • Requires basic computer skills and experience to include MS Word, Outlook and an electronic medical record (EMR).
  • Satisfactory completion of Chestnut’s background checking process.
