Overview

Chestnut Health Systems is actively seeking a committed Supportive Services for Veteran Families (SSVF) Case Manager to join our team. This pivotal role entails providing crucial support and guidance to veterans and their families, ensuring access to vital resources and services. As an SSVF Case Manager, you will play a fundamental role in fostering stability and promoting well-being within our veteran community across multiple regions, while facilitating essential connections with necessary services.

Responsibilities

In this role, you'll be responsible for various essential tasks. This includes ensuring accurate data entry, updating SSVF databases, and adhering to program and Veteran Administration policies. You'll directly provide services identified in the Housing Stability Plan, ensuring they are veteran-driven and align with program requirements. Moreover, you'll advocate for the recovery model of treatment, striving for trauma-informed care, empowerment, and respect. Facilitating outreach, referral linkage, and advocacy for recommended community resources is also a significant part of this role, ensuring that veterans and their support systems have access to essential services. Additionally, fostering positive interactions within Chestnut and the community to maintain a favorable program reputation is crucial. You'll participate actively in meetings, program development, and offer necessary support to team members when required. Furthermore, providing consultation to Chestnut staff, community providers, families, or others regarding referrals and progress made by veterans will be an essential part of your responsibilities. You'll also attend various events, such as Veteran Fairs, job fairs, and conduct street outreach to locate and assist eligible veterans. Ensuring the accuracy and maintenance of SSVF Veteran Charts, advocating for housing sustainability, assuming leadership roles as assigned, and promoting Chestnut’s culture of customer service excellence will be integral facets of your role. Confidentiality maintenance of all organizational information gained while performing job responsibilities is of utmost importance. Lastly, you'll be expected to perform other duties as assigned or required.

Qualifications

  • Minimum of a high school diploma with 10 years’ experience in human services, health care, or military service, or a combination thereof.
  • Bachelor’s degree in a related field with 5 years of relevant experience or a Master’s degree in a related area.
  • Basic computer skills including MS Word, Excel, Outlook, Adobe Acrobat, and EMR experience.
  • Valid driver’s license, private automobile insurance, and insurability.
  • Ability to obtain DCFS CANTS Child Abuse Clearance.
