Overview

Chestnut Health Systems is seeking a Community Health Navigator to join the Chestnut Family Health Center and make a positive impact on the community's health. As a trusted member of the community, you will play a crucial role in bridging the gap between clients, community-based organizations, and healthcare providers by assisting individuals in accessing appropriate mental and physical health services, housing, substance use treatment, employment and any other services that assist an individual to improve his/her quality of life. The community Health Navigator also provides screening, triage, crisis intervention, and referral to supportive services. This position will work in Granite City, Maryville, and Belleville.

Responsibilities

Serve as a reliable intermediary to reduce socio-economic and cultural barriers between clients and healthcare institutions. Establish collaborative relationships with clients through various mediums, including in-person, telephone, video calls, text, and IM. Utilize Telehealth and networking systems to connect clients to providers in real-time during off-site visits and assist clients in accessing digital health-related services. Maintain records, participate in outreach events, and stay informed about community resources. Uphold treatment philosophies such as patient-centered, trauma-informed, recovery-oriented, and strength-based approaches. Promote Chestnut’s culture of customer service excellence and ensure the confidentiality and security of organizational information.

Qualifications

  • High school diploma with 10 years’ experience working in the human services field, health care or another advocacy role; or a Bachelor’s degree with 5 years’ experience as stated above; Master's degree is preferred.
  • Community Health Worker certification obtained within six months from the date of hire.
  • Basic computer skills, including MS Word & Excel, email, and experience with an electronic medical record (EMR).
  • Valid driver’s license, private automobile insurance, and the ability to be insurable.
  • Trusted member of the community with the ability to act as a liaison to Medicaid population and health institutions.
  • Capability to develop public health seminars and navigate digital systems to reduce client barriers to accessing healthcare.
