As a full-time Behavioral Health Outreach Technician in McLean County, you’ll have the opportunity to be at the heart of our mission, working directly with individuals seeking care through our Behavioral Health Clinic. You’ll be the first point of contact for many clients, helping them navigate through our services, accessing vital community resources, and helping them take critical steps toward recovery. We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers. Responsibilities

  • Engage and Empower Clients: Build meaningful relationships through outreach and support, guiding clients as they take their first steps into our program.
  • Be a Trusted Guide: Conduct screenings and surveys to identify client needs and ensure they are connected to the right resources, both within Chestnut and in the community.
  • Data Collection and Reporting: Maintain database information and spreadsheets for program quality and compliance tracking.
  • Use Evidence-Based Approaches: Apply techniques like Motivational Interviewing (MI) and the Community Reinforcement Approach (CRA) to help clients overcome challenges and develop essential life skills.
  • Support Wellness: Work within a Recovery and Trauma-Informed Care framework to foster a sense of hope, respect, and empowerment for every individual.
  • Champion Client Success: Ensure timely and accurate documentation to track client progress, providing the best care possible.

Qualifications

  • 21 years or older
  • High school diploma (or equivalent)
  • Experience in human services/social services/community services and know how to work with adults in need of support
  • Requires moderate computer skills and experience to include MS Word, MS Excel, Outlook and electronic medical record (EMR).
  • Valid driver’s license, auto insurance, and a reliable vehicle
