Overview

A full- or part-time Case Manager works to promote integrated primary and behavioral health care. You will work with adults facing both mental health and physical health challenges. You will help clients by connecting them to services like therapy, medication support, and community resources. The goal is to improve clients' overall health and well-being by supporting their journey toward recovery.

Responsibilities

  • Complete paperwork like health assessments, progress notes, and visit notes.
  • Provide services by helping clients connect to therapy, medication training, providers, community support, or other services needed.
  • Support recovery by encouraging clients to take charge of their recovery journey. Use positive language to support hope, health, and wellness.
  • Work well with external agencies and support teams by building positive relationships with hospitals, doctors, courts, and community agencies.
  • Stay organized by keeping paperwork up-to-date and accurate, including attendance and housing documentation.
  • Make referrals by connecting clients to community resources like food pantries, doctors, and substance use programs.
  • Help improve services by suggesting ideas to improve services and assist with staff training or audits.
  • Be a team player by working well with internal multi-disciplinary team and offering guidance to new staff or interns.
  • Lead projects as assigned by your supervisor.

Qualifications

  • Master's degree

OR

  • Bachelor's degree and Certified Alcohol and Drug Counselor (CADC)

OR

  • Bachelor's degree in behavioral health or a related field

OR

  • Bachelor's degree in non-behavioral health and 2+ years of experience in behavioral health-related field

OR

  • High school diploma/GED and 5+ years of social service or peer experience

OR

  • High school diploma/ GED and Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS) in Illinois
  • Basic computer skills, including Microsoft Suite and electronic medical records.
  • Valid driver's license, car insurance, and insurability.

The anticipated starting pay for new hires for this position is between $21.00 per hour - $50,575.00 annual salary. There are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity.
