Overview

We are seeking a full-time Case Manager (Specialty Courts) to join our team in Granite City, Illinois. This role works Monday through Friday with a typical flexible schedule and may require evening hours to meet client needs. The Case Manager II plays a vital role in supporting individuals involved in specialty/drug court by providing case management, counseling, and care coordination services to promote recovery and successful program completion.

Responsibilities

  • Complete timely and accurate documentation, including assessments, treatment plans, IM+CANS, progress notes, and grant-required documentation in accordance with agency and funder standards
  • Conduct client assessments for drug court participants and determine appropriate levels of care with supervision
  • Provide individual, group, and specialized case management services, including transportation, home-based, and virtual support to reduce barriers to treatment engagement
  • Coordinate care with treatment teams, probation, referral sources, and community partners to ensure a continuum of care
  • Utilize evidence-based practices such as Community Reinforcement Approach (CRA), Motivational Interviewing, Seeking Safety, and Interactive Journaling
  • Support clients throughout treatment, including discharge planning and ongoing recovery support
  • Facilitate and monitor random drug screenings as required
  • Promote trauma-informed, recovery-oriented care emphasizing empowerment, wellness, and client self-direction
  • Provide referrals, linkage, and advocacy to community resources to meet client needs
  • Maintain strong collaboration with Madison County Probation and other stakeholders
  • Attend required meetings, court hearings, staffings, and trainings, including multidisciplinary team meetings
  • Provide services within an Immediate Access model, including crisis intervention and jail-based assessments when assigned
  • Demonstrate leadership by supporting team members and contributing to program development
  • Maintain confidentiality and uphold Chestnut’s standards of customer service

Qualifications

  • Bachelor’s degree with 5 years of supervised mental health experience and ability to obtain Certified Alcohol and Drug Abuse Counselor (CADC) certification within 2 years OR
  • Bachelor’s degree with active CADC certification in good standing in the State of Illinois
  • Experience working with families in community-based settings preferred
  • Willingness to complete Community Reinforcement Approach (CRA) training and obtain certification within 3 months of training
  • Valid driver’s license, current automobile insurance, and insurable driving record required
  • Basic computer proficiency, including Microsoft Word, Excel, Outlook, and electronic medical records (EMR) systems
  • Ability to work flexible hours, including evenings as needed

The anticipated starting pay for new hires for this position is between $21.00 - $22.00 an hour. There are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity.
