Chestnut Health Systems is seeking a compassionate, organized, and community-focused Community Access Navigator to support the Just Care for Families program serving Klamath and Lake Counties.
In this role, you'll help strengthen families by connecting them with essential resources, building partnerships throughout the community, and managing the Care Store, an innovative program that helps families access clothing, household items, family activities, and other basic needs.
Every day is different. Whether you're networking with community organizations, organizing donations, helping families overcome barriers, or supporting the clinical team, your work will make a direct and lasting impact.
